When creating an appeal online, please submit (by mail or hand-delivered) or upload all required documentation as referenced below.
Note: When filing an appeal to the SBOE by mail or hand-delivered, please reference the relevant petition form instructions for documentation requirements.

- Petition for Review of Valuation (DOR Form 82130).
- Personal Property Review of Valuation (DOR Form 82530).
- Petition for Review of Taxpayer Notice of Claim (DOR Form 82179C-1).
- Petition for Review of Taxpayer Notice of Claim - Personal Property (DOR Form 82179CP-1).
- Petition for Review of Notice of Change (DOR Form 82130NC).
- Petition for Review of Proposed Correction - Real Property (DOR Form 82179C).
- Petition for Review of Proposed Correction – Personal Property (DOR Form 82179CP).
